Please copy your diet journal notes from each day into the appropriate box. Please also indicate the day of the week and the date for each day’s-worth of data you enter.
Please list the total amount consumed for each food item you ate during the week you were keeping track of your intake. It would be helpful to list similar (in the same food group) foods together. For example, 3 C 2% milk 6 oz cheddar cheese 20 12-oz cans Mt. Dew 1 orange
Please fill in the Recommended Daily Allowances that are suggested for a person of your age, height, weight, and health. Also, for each nutrient, include the actual total amount you ingested for the week and your daily average. Indicate whether this amount was high, low, or about right as compared to what you should be getting (for example, you might not consider 1 g of Vitamin C to be “high” for you, even though that’s well above the RDA of 60 mg.). It is expected that you will have numbers for all nutrients for which data are available from the NAT Web site. Note that nutrients not found in NAT are indicated by a darker-colored background. Even though NAT does not list those nutrients, they are important nutrients, and you need to make sure you’re getting adequate amounts. If you were able to find numbers for those nutrients from other sources, please include as many of those as possible.
